Below is the intake document to print, fill out and bring with you!  No printer, no problem - just come 10 minutes early to your new patient appointment and you can fill them out there.

Health Profile Intake Form

Congratulations on taking the first step toward better health and healing through Chiropractic.  Your first visit may range from 30 minutes to 1 hour, this depends on complexity of you health history, if x-rays are needed questions you may have etc.  We use the Gonstead system to care for our patients, which is considered the gold standard. The Gonstead System is unique because it is complete and specific in the analysis, adjustment of the entire spine, and management of the patient from start to finish.
Therefore, it is important for you to know what to expect when you visit us. Read over the following procedures a new patient should expect to receive on their first and subsequent visits.

Health HISTORY

If your car breaks down, you explain the problems you are having to the mechanic and how you've been taking care of your vehicle. This is similar to your chiropractic health history.  First we will ask about what is currently happening in your health, ask more direct and specific questions if needed and then get an idea of your general health, lifestyle and past. All the information discussed with the chiropractor is considered when evaluating your spine and overall health.

Visualization

The doctors at Junge Family Chiropractic are experts at noticing subtle changes in posture, skin tone (when is the last time you looked at the skin tone on your back?), gait (walking) and movement which could indicate underlying problems. There is a lot of information that can be learned by looking at the "appearance" of the human body and structure.

Instrumentation

The instrument of choice here at Junge Family Chiropractic is the Nervoscope. The Nervoscope detects uneven heat on either side of the spine which can be indicative of local acute inflammation or chronic nerve pressure. This instrument is glided down the length of the spine and feels like two fingers moving down your back.  We also use a computerized version on the first day and at follow up exams to track progress.

Static Palpation

Palpation is the art of feeling things by hand.  In the chiropractic office this simply means assessing the spine with our trained hands. The doctors assesses tone imbalances, for sticky or swollen areas, tenderness, and other abnormal tissue changes that are associated with underlying problems.

Motion Palpation

This process is very similar to static palpation (see above) but instead the doctors move your vertebra in various specific angles.  This allows the doctors to identify any area of restricted motion or abnormal motion. An abnormal finding of spinal bone motion (vertebra) can mean misalignment. These misalignment can cause nerve irritation and lead to symptoms and other health challenges.

X-Ray Analysis

X-ray film allows the doctors to visualize your entire spine and structure which support them.  This is helpful in evaluating for any disease processes including degeneratvie disc disease, fracture, joint, and disc integrity, and any vertebral misalignment.

Adjustment

After completing a complete analysis of your spine and overall health, the doctors at Junge Family Chiropractic and Gonstead doctors worldwide are ready to deliver a hands on specific adjustment.  The focus of the Gonstead adjustment is to be as precise and accurate as possible when adjusting the spine.  Time is taken to ensure mechanically correct positioning, then a calculated thrust is delivered to produce the most effective and pain free adjustment possible.
